Freedom of Information Act (FOIA) Request

Copies of reports can be obtained through the Kankakee County Coroner's Office. Please provide a written statement which includes the decedent’s name, date of death, and your relationship or please fill out the FOIA form located at the bottom of this page. Please note, if you are requesting photographs the photography release form must also be completed and signed by the legal next-of-kin. Once the FOIA form is completed it can be either mailed to the Kankakee County Coroner’s Office at 3000 South Justice Way, Kankakee, Illinois, 60901, or e-mailed to lmjones@k3county.net.

Please include your return address along with a phone number where you can be reached. Once the request has been processed, you will be notified of the charges. Please allow 3-5 business days for processing. Reports can only be issued once the case has been completed. The release of photographs will be at the the discretion of the Kankakee County Coroner. Photos can only be released to the legal next-of-kin. If you are not the legal next-of-kin, please have the legal next-of-kin fill out the Release of Privacy Interest Form.

Per the State of Illinois, Public Act 096-1161, fees are as follows:

Autopsy Reports: $50.00

Toxicology Reports: $25.00

Miscellaneous Reports: $25.00

Photos: $3.00 per photo

Coroner's Report: $25.00

NOTE: Acceptable methods of payment are: cash, credit card, cashier’s check or money order. Please be advised there is a credit card transaction fee assessed by our credit card vendor.
